PLEASE PRINT THE ORDER FORM AND SUBMIT WITH THE SMALL AMOUNT  OF ASHES.

I need 1 tsp. of ash per SoulBurst. I will return all unused ashes with your pieces. The best way to send ashes is in a zip lock bag with your name on it in a padded envelope. UPS is the most secure way of shipping your ashes. USPS is also good. I keep a locked mail box for security.

Your Soulburst should arrive 2 to 3 weeks after I receive your package. I will email you and let you know that I received you package and when you can expect to get your SoulBurst back.

                     PAYMENT

Payment can be made by check or through PayPal. If including a check with your order make it payable to Michelle Kaptur. If you prefer to make your payment via PayPal. You may use the buttons on this page.

                       NOTES

Many family members and friends can request their own SoulBurst keepsake memento from a mutual loved ones cremains. You can still spread the remaining ashes in a special place.

There is a quantity discount if you order 3 or more SoulBursts with the same ashes.  

You may also choose to mix different ashes together in one SoulBurst.
